A neglected problem of developing countries: Noncystic fibrosis bronchiectasis

Abstract: Background:Bronchiectasis has been defined as the abnormal and permanent dilation of the bronchi. It is still an important problem in many developing countries. Aim:The aim of this study was to identify the chacteristics and underlying etiology of children followed with the diagnosis of noncystic fibrosis bronchiectasis. Materials and Methods:Children with bronchiectasis confirmed with high-resolution computed tomography were enrolled into the study. The data of the patients, including symptoms of the disease, age at the onset of symptoms, findings of physical examination, labrotory investigations performed in order to identify the etiology of bronchiectasis, etiology of bronchiectasis if found, radiologic findings and treatment modalities were noted. Results: Sixty-six children between 1 and 17 years were included in the study retrospectively. Forty-four of them were males (66.7%) and 22 (33.3%) were females. The most common presenting symptoms were cough (100%) and sputum expectoration (50%). An underlying etiology was identified in 44 (66.7%) of the study subjects. The four most common underlying causes were found as infections (21.2%), asthma (16.7%), aspiration syndromes and/or gastroesophageal reflux disease (9.1%) and immunodeficiency syndromes (7.6%), respectively. Conclusion:Identifying an underlying etiology will have a significant effect on the management of noncystic fibrosis bronchiectasis. Defining the cause of bronchiectasis may also decrease its incidence, progression and complications.

Abstract: The aim of this multicenter study was to provide insight into allergy knowledge and perceptions among the population.During the World Allergy Day, several allergy clinics conducted public meetings to encourage the awareness of and education in allergy. At the beginning, participants filled out a questionnaire to assess their knowledge about what is allergy and to determine by whom those symptoms are cared.A total of 256 participants (187 women/69 men, mean age, 31.2 ? 12.5 years) completed the survey. Of the 202 participants with symptoms, 58.9% had physician-diagnosed allergic disease. Among the 19 symptoms evaluated, 56.5% of the symptoms were recognized as related with allergy, and this increased in compliance with education level (r = +0.427; P < 0.001) but not with diagnosed allergy (P = 0.34). Sneezing was the most common symptom thought to be related with allergy-related symptom (77.5%), whereas loss of smell was the least one (37.9%). Participants were more likely to be cared by an allergologist (72.9%) followed by other specialties, when experiencing allergy.Increasing the awareness for allergic symptoms is the key not only for the diagnosis but also for the optimal treatment. Therefore, education is an important component of prevention and control of allergic diseases.
Questions of Culture in Distance Learning: A Research Review

Abstract: This paper reviews past research that focused on questions of culture in distance learning. Of specific interest are the studies that examined the influence of culture on students’ learning and engagement in asynchronous learning networks (ALNs). The purpose of this review is three-fold: to present the state of knowledge concerning the questions of culture in distance learning, to highlight important methodological issues that past research has left unresolved, and to provide practical insights into teaching culturally and linguistically diverse online communities of learners. For these purposes, 27 studies are examined and the findings are reported under the following two categories: What do studies focusing on questions of culture in distance learning tell us? What implications do they suggest for practice and future research? Also, the paper provides methodological insights for researchers who wish to investigate the cultural dimensions of distance learning in future studies.
EFFECT OF CRUDE PECTINASE ON APPLE JUICE QUALITY CHARACTERISTICS BY DESIRABILITY APPROACH

Abstract: The present study analyzed the efficiency of crude pectinase produced from Bacillus subtilis using hazelnut shell hydrolysate during clarification of apple juice. Response surface methodology (RSM) based desirability analysis was carried out to optimize the enzyme load, temperature, and time with respect to lightness, turbidity and clarity. The apple juice clarification was performed at different enzyme loads (0.3-0.7%), pH (4-7), temperature (30-50 oC) and time (2-6 h). The RSM results revealed optimum clarification conditions of 0.3% (w/v) enzyme load, 45.8 oC temperature, and 2 h of time resulting in 70.40 lightness, 49.44 turbidity and 88.33% clarity. To confirm the validity of RSM models, RMSE values of L value, turbidity and clarity were calculated as 4.65, 5.28 and 9.15, respectively. As a conclusion, maximum clarity at a low enzyme load and time makes the enzyme useful for juice industry

Comparison of Fluoroscopic Voiding Cystourethrography and Voiding Urosonography in the Diagnosis of Vesicoureteral Reflux in Children

Abstract: Purpose: The goal of the study was to investigate the success rates of voiding cystourethrogram and voiding urosonography in patients who were scheduled for further investigation vesicoureteral reflux. Material and Methods: This study included 48 patients who were evaluated for vesicoureteral reflux and planned for voiding cystourethrogram. The patients were evaluated for gender, age and nephrologic problems which causing for plan voiding. Results: Of the 48 patients in the study, 34 were female and 14 were male. Vesicoureteral reflux was detected in 18 of 48 patients. All renal-ureter units were evaluated for vesicoureteral reflux and 91.3% with voiding cystourethrogram and 73.9% with voiding urosonography were detected in the renal -ureter unit. The coherence rate of both methods was found to be 73.6% and it was shown that there was no statistically significant difference between the two methods for the detection of vesicoureteral reflux. When the cases of grade I vesicoureteral reflux were excluded from the study, the consistency rate of both methods increased to 87.6%. There was no statistically significant difference between the methods for detecting vesicoureteral reflux in renal ureter units (p=0.146). Conclusion: Voiding urosonography can be used as an alternative radiation-free imaging method for selected cases
“Ideal Medical Educator”: Cognitive Structures of Medical Faculty Students

Abstract: Objective: Several factors influence the perception of medical school students regarding the concept of medical educator. The most important of these is the medical educators in the faculty where they are studying. In this context, in this research, it was aimed to determine the qualifications of 'ideal medicine educator' in the cognitive structures of the students and to design a model according to these qualities. Methods: In this research designed by exploratory pattern from mixed methods, 217 students who are attending different grades in the medical faculty of a state university in the Central Anatolia Region include the study group. The data were obtained through the 'repertory grid', which is one of the structured interview techniques. Content analysis and grid similarity formulas were used in the analysis of the data. Results: Findings indicate that there are 1,627 valid cognitive structures (only positive qualities) representing ideal medical educator qualities, and that these structures are separated into 5 major and 24 subcategories. The first three most frequently repeated cognitive structures are classroom management skills, compassion and assessment and evaluation skills. The main categories constitued by being classfied cognitive fiction are pedagogical competence, individual maturity, communication skills, innovative spirit / aversion and other characteristics. Conclusion: The results revealed that the nature of the medical educator should be explained as a whole rather than just a few qualities. These qualities vary from the teaching profession to the dominance of the field (medical) knowledge, from courtesy and sincerity. Based on the qualities obtained in the research, the ?Circular Medical Educator Model? consists of six components and each of the 24 components related to each other. Each component is separated into its own structure. The two most important essential qualities of the medical educator in the model are 'Pedagogical Competence' and 'Individual Maturity'. The created circular model allows to evaluate the medical educator as a whole and all components and structures can be used to explain the qualities of medical educators in a systematic relationship pattern
Discrepancy in the lower arch perimeter in patients with a unilateral cleft lip and palate: orthodontic model analysis

Abstract: Objective: This study aimed to assess the lower arch length discrepancy in a group of patients with unilateral cleft lip and palate (UCLP). Materials and Method: Pretreatment dental casts and panoramic radiographs of 23 patients (aged 9–19 years) with a nonsyndromic complete UCLP, without having large restorations/crowns, tooth agenesis, impacted or supernumerary teeth in the lower arch, and previous orthodontic and/or prosthetic treatment, were evaluated. All patients underwent lip and palate repair. Lower arch discrepancies were determined using the Hayes-Nance analysis. Panoramic radiographs were used to estimate the size of permanent premolars for the patients with late mixed dentition. The positive discrepancy defined diastema, whereas the negative discrepancy defined crowding in the lower arch. Descriptive statistics were presented using frequencies and percentages, and the differences were evaluated using the binomial test. Results: One out of 23 patients had no discrepancy in the lower arch. For the remaining patients (n = 22), the prevalence of diastema was 47.8% (n = 10), with a mean value of 3.6 ± 1.9 mm, and lower arch crowding was observed in 52.2% (n = 12), with a mean value of –2.9 ± 1.4 mm. No significant difference was found between the prevalence of crowding and diastema (p = 0.832). Conclusion: In patients with a UCLP, diastema can be encountered approximately at the same frequency as crowding in the lower arch. Crowding was at a low to mid-level
Radiographic Examination of Talocrural Joint Morphometry in Individuals with Normal Joint Anatomy

Abstract: Aim: The traumas of the talocrural joint are very common in population. Therefore; the anatomy and morphometry of this joint has a great importance. The aim of this study was to examine the morphometry of talocrural joint in large series of patients according to age and sex in order to add some new knowledge to the literature.At the same time, according to the results of the research, it is aimed to give a way for field-specific prosthesis production to the prosthetic producers. Material and Method: In this study; the talocrural joint radiographs of 274 anatomically normal patients (134 males, 140 females) were examined. The morphometric measurements of the defined parameters were done and their statistical analysis were performed according to age and sex. As far as we know, from our parameters; the mediolateral diameter of tibia (TML), the mediolateral diameter of fibula (FML), surface area of trochlea tali (TTA) and SIGMA angles were firstly defined in this study. Additionally; the innermost distance of talocrural joint (ATI), the outermost distance of talocrural joint (ATD), the angles in between the medial malleolus and talus (DELTA), the lateral malleolus and talus (ALFA) and the sagittal length of trochlea tali (TSU) were also measured. Results: In the comparison of TML, FML, TTA, ATI, ATD measurement sand ALFA angle in both sexes; they were found to be higher in males than in females. However; TSU and SIFGMA angle were found to be higher in females, when compared with males. The DELTA angle was nearly the same in both sexes. Conclusion: These morphometric analyses were performed in a huge number of anatomically normal patients’ radiographs and therefore; we believe that the study will add a new knowledge to the literature, will help to the clinicians who are interested in the ankle joint and will be useful for the companies who are working on implant technology
